Overview: Accident investigation/root cause analysis is a key element of safety and health activities, and should be considered an operational priority. Many companies already have programs established for accident investigation/root cause analysis, but many of these programs are ineffective.  It is difficult to sustain an Accident investigation/root cause analysis Program without the proper controls and adequate review of investigation activities to ensure continued quality.
